Whether an outbound request is held for a person’s approval, using the agent’s hold rules.

Rules
- The Bouncer decides; the egress enforces.
- A held request stores a reviewed copy, its hash and a size-limited body.
- The limits are HELD_BODY_LIMIT and HELD_STORED_BODY_LIMIT.
